ABSTRACT

In the clinical practice of rheumatic immune diseases in traditional Chinese medicine (TCM),it`s still unclear about the dominant diseases and breakthrough points. It`s urgent missions to formulate TCM diagnosis and treatment guidelines widely recognized and integrated by traditional Chinese medicine and Western medicine. In order to clarify the dominant diseases and breakthrough points in rheumatism,China association of Chinese medicine initiated a research group covering experts in the field of rheumatism of traditional Chinese medicine and Western medicine. Based on questionnaire survey and on-site discussion,experts had reached the following consensus. Evidence-based medicine research using modern medical methods and scientific methods should be carried out to provide objective clinical evidences. "Four mutuality" were put forward as the basis for the work of integrated traditional Chinese and Western medicine,that is the mutual communication using the exchangeable context,the mutual explanation using common theories,the mutual certification using common standards,and the mutual integration using common means. Key works should focus on solving refractory rheumatism in the future. In terms of dominant diseases and breakthrough points,this paper introduces 21 breakthrough points in 6 dominant diseases,including rheumatoid arthritis,ankylosing spondylitis,Sjogren's syndrome,hyperuricemia and gout,systemic lupus erythematosus and fibromyalgia syndrome. Advice on this discussion can provide valuable references for developing the treatment scheme of rheumatism with TCM and integrated Chinese and Western medicine and clinical practice and scientific research.

ABSTRACT

Objective:To investigate the effect of Huoxue Jiedu Runzao prescription on the morphology, apoptosis, and function of submandibular gland in the mouse model of Sjögren's syndrome (SS) and its functioning mechanism, we analyzed the expression of the apoptosis inhibitor Survivin in the submandibular gland cells of SS mice. Method:Female BALB/c57 mice were selected as the normal group. The naive non-obese diabetic (NOD/Ltj) female mice were selected as the SS model, which were randomly assigned into the model group, Paeoniae Radix Alba total glucosides capsule (0.234 g·kg<sup>-1</sup>) group, and low-, medium-, and high-dose (15.6, 31.2, 62.4 g·kg<sup>-1</sup>, respectively) Huoxue Jiedu Runzao prescription groups. Each group had 15 mice. The morphological and functional changes of submandibular gland and the Survivin expression were observed and measured after 8 weeks of drug intervention. Survivin expression was determined by immunohistochemistry (IHC), Western blot, and reverse transcription-polymerase chain reaction (RT-PCR). Result:Compared with normal group, salivary flow and submandibular gland index in model group were significantly decreased (<italic>P</italic><0.01), and histopathological score of submandibular gland was significantly increased (<italic>P</italic><0.01). Western blot showed that Survivin protein expression was significantly decreased (<italic>P</italic><0.05). IHC showed that, Survivin mRNA expression was significantly decreased (<italic>P</italic><0.01), and RT-PCR results showed that Survivin mRNA expression was significantly decreased (<italic>P</italic><0.01). Compared with model group, salivary flow and submandibular gland index of mice in Huoxue Jiudu Runzao prescription groups and Paeoniae Radix Alba total glucosides capsules groups were significantly increased (<italic>P</italic><0.05), histopathological score of submandibular gland was significantly decreased (<italic>P</italic><0.05), IHC results showed that Survivin expression was significantly increased (<italic>P</italic><0.01). Western blot and RT-PCR results showed that Survivin protein and mRNA expression of Huoxue Jiudu Runzao prescription high-dose group and Paeoniae Radix Alba total glucosides capsule group were significantly increased (<italic>P</italic><0.05). Conclusion:Huoxue Jiedu Runzao prescription can improve the secretion function of submandibular acinus, increase the submandibular gland index, and saliva secretion of SS mice by up-regulating survivin in submandibular gland cells of SS mice.

ABSTRACT

Objective The HOTAIR gene is closely related to pannus formation in rheumatoid arthritis (RA). This study aimed to construct and screen fibroblast-like synoviocytes in human RA (HFLS-RA) stably overexpressing lncRNA HOTAIR, and to pave the way for further study of the role of lncRNA HOTAIR in the pathogenesis of RA. Methods LncRNA HOTAIR was cloned and linked to the PMT406 vector digested by BamHI-HF-HF and XhoI. The constructed plasmids were sequenced, identified and then transfected into 293T cells to pack lentivirus. The HFLS-RA cells were infected with the recombinant and empty vector lentiviruses, and purinomycin was employed to screen the lncRNA HOTAIR-overexpressed and control cell lines. The total RNA was extracted from the blank, negatively transfected and overexpressed cells by Trizol, and the cDNA obtained by reverse transcription was amplified by qPCR, followed by determination of the expression of lncRNA HOTAIR. Results The relative expression of lncRNA HOTAIR was significantly higher in the overexpression group than in the blank control and negative transfection groups (30.329 ± 3.860 vs 1.001 ± 0.048 and 0.892 ± 0.247, P 0.05). Conclusion The HFLS-RA cell line stably overexpressing lncRNA HOTAIR was successfully constructed, which has provided some experimental evidence for further investigation of the role of lncRNA HOTAIR in the pathogenesis of RA.

ABSTRACT

Objective To evaluate the clinical value of color doppler flow image (CDFI) and three-dimension color power angiography(3D-CPA) in the diagnosis of thyroid nodules. Methods A total of 62 pathologically confirmed thyroid nodules of 30 patients were analyzed. All the patients underwent ultrasound examination and operation in the Second Affiliated Hospital of Harbin Medical University between January, 2008 and May, 2009. Both CDFI and 3D-CPA were performed before the operation. All the nodules were divided into three groups including nodular goiters, thyroid adenomas and thyroid cancers according to the pathological results. The hemodynamic features and the vascular morphology characteristics of nodules in different groups were compared. Results 3D-CPA showed that blood vessels of nodular goiters commonly distributed surrounding the mass and the inner vessels were thin and regular, thyroid adenomas were bulb-shaped and netted structure, and malignant thyroid nodules displayed distorted and irregular distributed vessels. Peak systolic velocity (PSV)of the three groups were (39.43±11.17a), (46.39±12.98) and (65.17±9.23)cm/s, respectively. Resistance index(RI) of the three groups were (0.32±0.08), (0.41±0.06) and (0.69±0.07)cm/s, respectively. Both PSV and RI in malignant thyroid nodules were higher than in nodular goiters and in thyroid adenomas and the difference were statistical significant (all P < 0.05). The blood flow grade of malignant nodules was also higher in malignant nodules than in other two groups(χ2 = 17.11, 12.79, 23.05, 15.41, P< 0.01). Conclusions CDFI and 3D-CPA could visually demonstrate the characteristic and distribution of the inner and outer blood vessels, display the vessels structures, and they are benefit the differential diagnosis of thyroid nodules.

ABSTRACT

<p><b>OBJECTIVE</b>To study the prevalence of snoring and obstructive sleep apnea-hypopnea syndrome (OSAHS) and its high risk factors in Guangxi Zhuang Autonomous Region.</p><p><b>METHODS</b>From January 2003 to March 2005, a total number of 11,163 persons aged > or =14 years Zhuang minority living in Guangxi (from Guinan, Guizhong and Guibei) were surveyed. Questionnaire was administered to draw information. Polysomnography(PSG) and in-home polygraphy were performed on participants being studied and who had reported snoring.</p><p><b>RESULTS</b>Among all the surveyed people, 2940reported snoring with a prevalence of 27.3%. 448 (320 males and 128 females) people reported OSAHS with prevalence as 4.3% (5.9% in males and 2.5% in females). From 14 to 60 year olds, the prevalence of snoring and OSAHS increased with age. Among those above 60 years of age, both the prevalencerates ofsnoring and OSAHS werereduced with age. Among all the study population, 260 (21.6%) had habitual OSAHS a nd 188 (10.8%) hadoccasional OSAHS. The high risk factors of OSAHS were: position during sleep, disease of nose, drinking alcohol, smoking, gender, body mass index (BMI) and age.</p><p><b>CONCLUSION</b>The prevalence rates of snoring and OSAHS were 27.3% and 4.3% respectively. From 14 to 60 years of age, the prevalence rates of snoring and OSAHS were increasing with age while from 60 years of age on, the prevalence of snoring and OSAHS reduced with age. The prevalence rates of snoring and OSAHS in males were higher than females. The high risk factors of OSAHS were position during sleep,disease of nose, drinking, smoking,gender, BMI and age.</p>
